Live Well Greenwich

From food to money to socialising, all sorts of things in life affect our health and wellbeing. Live Well Greenwich offers signposting and support to get you the help you need to improve your health.

The Live Well team at Charlton Athletic Community Trust (CACT) can help with:

  • Eating well
  • Moving more
  • Social and community activities
  • Stopping smoking
  • Drinking less
  • NHS Health Checks
  • Housing needs
  • Money, debt and benefit advice
  • Employment and training
  • Support for managing your long term condition

Support is available through our free Live Well helpline, where you can connect with local, trained health and wellbeing advisors.

The Live Well Line is commissioned by the Royal Borough of Greenwich and delivered by CACT, 6 days a week. It has humble beginnings, originally commissioned to support the Greenwich Get Active, physical activity programme and Kick the Habit smoking cessation Programme.

Over the years, it has expanded to offer a single point of access; an all inclusive service to support residents and refer them to a range of community programmes across the borough. The Live Well Line now acts as the foundation for all of our health improvement work at CACT.

CACT Health Improvement Advisors are part of a network of local Live Well Coaches, who are able to provide signposting and support to help you live well. Our health advisors develop and maintain relationships with clients, motivating them towards leading healthier lifestyles and refer or book them onto any services they may need.

Live Well Coaches can offer up to six 45-minute sessions over the phone. To self-refer for the service, call or fill in the online referral form.

To talk to a Live Well Coach call the Live Well Line on 0800 470 4831.

To find health, wellbeing, social care and community information and services local to you, head to the Greenwich Community Directory.

Live Well Greenwich is available to any residents in the Royal Borough of Greenwich who feel they could benefit. Please note this service is not for patients requiring mental health crisis support.
